  4. Select Effective Date: Is always in the current pay period or future, no previous dates before current pay period will be allowed.
  5. Select Rate Type: New Pay Rate ($) or Dollar Increase ($) or Percent Increase (%)

  1. Answer question 1 in detail
  2. Answer Question 2, may ask HR for the date
  3. Answer Question 3, if marked no, do not proceed and see HR, if marked yes then proceed to next step
  4. Answer questions 4 & 5 and proceed
